First Connect

First Connect is operational in Northern and Belfast Trust areas, offering practical and emotional support at difficult times to those aged 65 and over. Difficult times can include bereavement which can result in feelings of loneliness and vulnerability. The coordinators within this person-centred programme offer both group and bespoke one-to-one support depending on each individual’s wants and needs. Staying Sharp

The Staying Sharp programme offers emotional and practical support, as well as advice, education, connecting with others and fun!  It aims to support those people living in Ballymena area who have been told they have a mild cognitive impairment or diagnosed with dementia.

Times like these can impact on a person’s sense of self confidence or self-esteem and quality of life.  Through Staying Sharp, we are there to support individuals aged 60 and over. We want them, and their carers, to be make sense of what is happening in their lives and to help them to make choices and access new opportunities.

 

What can you expect from Staying Sharp

. Support to complete benefit forms.

. Befriending service with support of volunteer.

. Online groups including singing for health, relaxation, reminiscence.

Living Well Moyle

Living Well’ is about people, place and purpose: the focus is on the person, understanding their aspirations and relationship with their community; the shared purpose is shaping everything around a holistic person centered and coordinated approach to improve the quality of life for people aged 18 and over; by helping them identify ways to build their self-confidence, self-determination, resilience and self-reliance; and by providing practical support to help them achieve their aspirations.

Individuals, who are living with a chronic condition, are supported by a Living Well Coordinator, who is part of the multi-disciplinary team within a GP practice, and a ‘guided conversation’ with individuals is undertaken. This enables the individual to identify their goals and, with the coordinator’s support, agree a management plan that is delivered on their behalf by statutory and community/voluntary sector services.  A key element of the service is the support of a network of volunteers, to help individuals become more physically and socially active within their community.
